Your sources.list file contains entries for Ubuntu 14.04 however you
have the lsb-release package from Ubuntu 16.04. This has confused the
release-upgrader and caused it to be unable to calculate the upgrade.
While there may be other packages from Ubuntu 16.04 also installed on
your system at a minimum please reinstall the version of lsb-release
from 14.04 and try the upgrade again. Thanks and good luck!
