Fixed in Ubuntu 16.04.4
NVMe disks are displayed as "NVMe disks" instead of Unknown.

Bug description:

  Ubuntu installer shows NVMe disks as 'Unknown' as shown in the attached screenshot.

  Reproducible:
  Always

  Steps:
  1. Setup a server with at least one NVMe disks
  2. Start Ubuntu-16.04.3 Server installation
  3. Notice that the NVMe disk is listed as 'Unknown' by the installer.

  Additional Info:
  * Attaching the screenshot.

  * During installation, udevadm does not have MODEL defined for NVMe
  disks.

  * I can see udevadm having MODEL info on booting into OS post-
  installation. Hence I think initrd requires some changes w.r.t to
  udev. Please correct me if I am wrong on this suspicion.
