I had exactly the same problem. Indeed removing the lines in the other files solved the issue!
I don't think I have followed the same guide but some other guide (don't remember which on exactly) a few years ago which roughly suggested the same.

  Wrong WARNING Option 'hash' missing in crypttab

Status in cryptsetup package in Ubuntu:
  Invalid

Bug description:
  I have installed kubuntu 18.10 on an encrypted partition.

  I have written a /etc/crypttab
  CryptDisk UUID=c9deed35-610e-4328-b8e7-079e95d43f76 none luks,hash=sha256,tries=3

  At each boot, I get this warning "cryptsetup: WARNING: Option 'hash' missing in crypttab".
  I tried with and without the hash=sha256 but the warning does not change.
