It's possible you had a bad write to your media. When having such
problems please try the option to check the integrity of the image from
the installer boot menu, and if this confirms that the media is corrupt,
try re-writing and/or re-downloading it.

Title:
  installer crash with "[Errno 5] Input/output error"

Status in ubiquity package in Ubuntu:
  Invalid

Bug description:
  Trying to install Ubuntu 18.04 from live USB over previous distro
  (OpenSUSE Tumbleweed), but ubiquity installer crashes with above error
  at start of partitioning (but after re-writing partition table?).

  I am unable to close the error dialog, and have access to the mouse,
  but cannot close applications or shutdown from GUI.  Switching to
  terminal (Ctrl-Alt-F2) and running `top` to find the PID for
  `ubiquity`, then killing this process gives me access to my GUI with
  Ubiquity closed.

  I initially tried to preserve my existing `/home` partition, and was
  no longer able to boot from the hard disk, but booting from the USB I
  could still mount the original partition.  Tried again, but this time
  asking to erase the entire disk and install with LVM support, but I
  got the same error, but no longer have access to my `/home` partition.
