Things start to go wrong with:
Sep 12 12:19:03 ubuntu kernel: [ 1061.268314] sd 2:0:0:0: [sdb] tag#0 FAILED Result: hostbyte=DID_OK driverbyte=DRIVER_OK cmd_age=2s
Sep 12 12:19:03 ubuntu kernel: [ 1061.268349] sd 2:0:0:0: [sdb] tag#0 Sense Key : Not Ready [current]
Sep 12 12:19:03 ubuntu kernel: [ 1061.268361] sd 2:0:0:0: [sdb] tag#0 ASC=0x4 <<vendor>>ASCQ=0xff
Sep 12 12:19:03 ubuntu kernel: [ 1061.268370] sd 2:0:0:0: [sdb] tag#0 CDB: Write(10) 2a 00 01 dc d9 b0 00 00 70 00
Sep 12 12:19:03 ubuntu kernel: [ 1061.268376] blk_update_request: I/O error, dev sdb, sector 31250864 op 0x1:(WRITE) flags 0x4000000 phys_seg 14 prio class 0
So it crashed because the disk was no longer available to the kernel.
Since this is a VM, we should expect this was not caused by a hardware
failure.
Were changes being made to the VMWare configuration at the time? Is
this crash reproducible for you?
