Thank you for taking the time to report this bug and helping to make
Ubuntu better.

Bug reporting is about finding & fixing problems thus preventing future
users from hitting the same bug.

I suspect a Support site would be more appropriate, eg.
https://answers.launchpad.net/ubuntu. You can also find help with your
problem in the support forum of your local Ubuntu community
http://loco.ubuntu.com/ or asking at https://askubuntu.com or
https://ubuntuforums.org, or for more support options please look at
https://discourse.ubuntu.com/t/community-support/709

Your provided details highlight installation media issues, thus installs
from bad media shouldn't be trusted anwyay. Did you validate your ISO
prior to write to media? and explore why your media fails check?

Either way, messages such as

[ 108.311200] sd 2:0:0:0: [sdb] tag#26 FAILED Result: hostbyte=DID_OK driverbyte=DRIVER_OK cmd_age=2s
[ 108.311206] sd 2:0:0:0: [sdb] tag#26 Sense Key : Medium Error [current]

[ 108.311225] Buffer I/O error on dev sdb1, logical block 267, async page read

highlight issues related to your hardware, thus I've marked this bug
report as incomplete and suggest you seek support.
