[Bug 2023584] [NEW] enforce format of rootfs and /boot

Public bug reported:

It's possible today to configure an install where the rootfs, or /boot,
are shared from a previous or current install.  We cannot reliably
support that configuration.

Subiquity shall enforce and configuration time, as part of the minimum
requirements to leave the storage screen + autoinstall equivalents, that
the destination for the rootfs is to be formatted.  If a distinct /boot
is in use, the same requirement applies.
