Title:
  The 'snap revert' hint displayed on upgrade is misleading

Status in ubuntu-release-upgrader package in Ubuntu:
  Fix Released

Bug description:
  During a dist-upgrade we get those messages displayed when refreshing
  a snap to another channel

  ```
  snap-store is now tracking 2/stable/ubuntu-24.04

  If needed, you can revert to your previous revision with:

  $ snap revert snap-store
  ```

  The revert command suggested will restore the revision which was in
  use before but not change back the channel, which means the snap will
  be autorefreshed again to a revision of the new channel the next time
  a build is published there. That's a confusing user experience.
