This bug is **not** a regression from 24.04.1, I can reproduce it using
Kubuntu 24.04.1 installation media as well.

Title:
  Fails to detect an operating system with /boot on a separate partition

Status in os-prober package in Ubuntu:
  New

Bug description:
  OS: Kubuntu 24.04.2 RC
  Hardware: Kubuntu Focus Ir16

  Steps to reproduce:

  1. Install Kubuntu 24.04.2 on a UEFI machine, using manual partitioning and setting a partition layout with a /boot/efi, /boot, /, and /home partition. Make sure the / partition is large enough to hold at least two Kubuntu installations since we'll split it in half later on.
  2. Install Kubuntu 24.04.2 to the same machine, using the "install alongside" feature and splitting the root partition made earlier. Preferably use a different language than normal to make the two installations easy to differentiate.
  3. Reboot after the second installation.

  Expected result: Boot menu appears allowing you to choose between the
  two Kubuntu installations.

  Actual result: Boots directly into the second installation, first
  installation appears to "vanish".

  More information: Enabling os-proper in /etc/default/grub and running
  `sudo update-grub` in the second installation does NOT add the first
  installation to the boot menu.
