Title:
Removing installation media (on recommendation) causes bad sector on
  USB HDD

Status in ubiquity package in Ubuntu:
  Won't Fix

Bug description:
  Ubuntu Version: 16.04
Not sure about the Package name or version.

  Steps to Reproduce:
!!!!!!Caution: This will make your USB drive unable to be a bootable linux (forever)!!!!!!

  1. Make a bootable usb drive installing ubuntu 16.04 on it.
  2. Now boot from this usb drive as if you are going to install it.
  3. Select Try ubuntu without installing.
  4. Try for a little while and then restart.
  5. While restarting there will be a note saying 'Remove the installation media and press Enter'

  Results:
  1. (As far as I could presume) while removing the installation media (usb drive) and pressing enter, the media was still in use or not ready to be removed yet, but the text already appeared saying 'Remove the installation media and press Enter'.

  2. After removing the media, use your previous OS which you were
already using, and plug in the USB drive, you will find your USB drive
at a size of 2/3 MB. You can regain the actual storage by
repartitioning the USB drive, but probably there will be a bad sector
created and you will never be able to boot from that USB drive again.

  3. I never used to remove the usb while the live linux still running,
but the latest 16.04 said me to remove and I thought it was ready to
be removed, otherwise I would shut down the PC and then remove the
USB, then again power up the PC. but because of that message, I lost
two of my USB drive's booting capacity. they now have a bad sector and
  'casper/squashfiles.fs' can't be loaded.

I will soon attach a screenshot of the error message.

Expected Results: Removing the media should not have caused this error
/ the message should have never appeared.
