Title:
release notes goes to wrong source

Status in ubuntu-release-upgrader package in Ubuntu:
  Confirmed

Bug description:
The Release notes page on the do-release-upgrade -d says to go to
  https://discourse.ubuntu.com/t/resolute-raccoon-release-notes, but you
will notice this link says that the patch notes have moved to
documentation.ubuntu.com/release-notes/. Wouldn't it be better to just
send you directly to the documentation instead of going through an
intermediary page that doesn't tell the end user anything?

  1) Ubuntu 24.04.4 LTS
  2) ubuntu-release-upgrader-core:1.24.04.28
  3) I expected the link to the release notes to send me to the new release notes page
  4) I clicked the link, and it send me to a secondary referral link

I don't think this is a blocker or anything, but perhaps it can be
fixed before official migration opens up? Should just be a single line
  change, I think.
