I also have this problem on a G4 with another hardware USR 56K Voice Fax Modem.
The stack dump, sig#, and Last Syscall were identical. Removing the modem fixes
the problem (of course). Booting with a 2.4 kernel works fine, and the modem
works great (have been using it as answering machine for years). I recompiled
the kernel with serial as a module and receive the message "WARNING: Error
inserting 8250 (/lib/modules/2.6.10enrock.2/kernel/drivers/serial/8250.ko): No
such device" when trying to "insmod 8250". I then recompiled the kernel with the
ubuntu sources (2.6.10), but replaced the serial driver with pristine (from
kernel.org) version (also 2.6.10), and now everything works. I guess this means
the problem is related to a debian or ubuntu patch that is being applied. I have
also verified the problem occurs with the ubuntu 2.6.11 sources.
Does this mean that no PCI modems (or PCI serial ports) will work with ubuntu on
a G4 (or possibly and Mac) with the 2.6 kernel?
