Thomas, thanks. the only real workaround is to perform a
netboot/netinstall.

It appears that this bug is strictly related to some OBP versions that
do not open/close/release the cdrom properly to silo when performing
"boot cdrom".

The fix we did introduce does some special casing to that, but we might
have not catched them all.

If you all guys could please provide me with exact model and OBP
version, we can probably figure out some more.
