[Bug 43305] USB stops working on Toshiba m45-s165

Public bug reported:

During normal use all USB ports on machine stop responding.  There are
no messages in dmesg.  Removing devices and reattaching has to affect,
and does not show up in dmesg.  Happens randomly, as far as I can tell,
on ever instance of using Ubuntu.  The only fix I can find is to reboot,
but I'm not a guru, either.
