I should add that wpa has worked intermittently.  When I first installed
and configured ndiswrapper, I had wpa until I rebooted.  Then, it would
only connect on an open network.  WEP was given as a configuration
option, but even if I changed the encryption on the router to WEP, ndis
does not connect.  I then completely removed ndiswrapper and all
configuration files, and reintalled.  Once again I had wpa until the
computer went to sleep.  When it woke up, once again I could only
connect on an open network.  WEP is again listed as an encryption option
(I did not test it again) and I can connect on an open network.

output of ndiswrapper -l:

rt2500 : driver installed
        device (1814:0201) present (alternate driver: rt2500)

output of ndiswrapper -v

utils Error: no version specified!
driver version:        1.38
vermagic:       2.6.20-13-generic SMP mod_unload 586 

not sure if the error message indicates a problem?  ndiswrapper_utils
shows as installed (latest version)
