Ok, one more update. I found something on the ndiswrapper mailing list archives:

Apparently, the way the hardware is, it requires a power off to reset
the firmware. If you leave it in an reboot, it will not properly come
back up. So, the workaround is to do a shut down if you want to leave
the dongle in the computer. If not, you have to remove the dongle and
put it back in when you reboot. I don't think it works properly if you
leave it in while it is booting up, you need to:
sudo modprobe -r ndiswrapper
sudo modprobe ndiswrapper
to reset it.
