mdadm:
 - changelog missing bug number
 - debian/initramfs/init-premount: needs new name for add_mountroot_fail_hook_d() (see above)
 - OK otherwise, although large patch which needs thorough testing
 
Verification should include dpkg-reconfigure, booting in both modes with degraded and non-degraded array.
