I've found in my case that there is a hardware fault on the motherboard,
an Asus A7M266D, whereby two of the three 32-bit PCI slots have some
kind of problem. I originally tried the FastTrax TX2000 in those slots
since there is a SATA controller in the first. When I swapped things
about I discovered the SATA controller also disappeared.

I then compared using another A7M266D motherboard which performs fine.

Once I get time I'll do some more comprehensive checks to be sure the
hardware fault is the cause of this report and if so close it as
Invalid.
