Your failure was due to a buggy hook in the iscan (or possibly iscan-
data) package. See http://bugs.debian.org/cgi-
bin/bugreport.cgi?bug=586554 for full details on this. Ubuntu doesn't
ship iscan, so we can't fix this directly, although I believe that the
upstream maintainers are working on a fix and should have an update
ready soon.
In the meantime, the new version of initramfs-tools at least makes it
clear which hook is responsible.
