@Jonathon (or anyone else who wants to debug):

The issue now seems to be that the webcam won't work when plugged in a
second time unless the module is reloaded. Can you run the same test
Colin asked for in comment #13, but this time I want you to plug in the
camera twice to trigger the bug. The second time hopefully will output
some information to kern.log that will help us determine what is going
wrong when we see the failure. Attach the new kern.log file here when
you've got it.
