Jeremy: Thanks for the follow up. I did diagnose the issue eventually and
it had to do with a too small /boot partition. Unless I resize that
partition, I can only have 1 previous kernel version still installed. If I
run out of disk space in /boot the results can be a little unpredictable
beyond knowing that "something will break".
The exact issue in this instance was that I had manually deleted some old
kernel images rather than uninstalling them (doh) and when I went to
upgrade, it couldn't find the old files.
