Public bug reported:

Binary package hint: initramfs-tools

I did an install using the 2010-03-26 lucid-alternate-amd64.iso daily
inside a KVM/libvirt virtual machine. I created a VM with 4 virtual
disks (8GB in size). Each disk was partitioned into one 500M partition
and a second partition taking the remaining space. I put /boot and
/dev/vda1. I assembled /dev/vd[abcd]2 into a mdadm RAID10 array, and
then used LVM to split /dev/md0 into a logical volume for /, and another
for /home/.

After the install completed, booting into the VM fails with a "Gave up
waiting for root device" message, and I'm dropped into a BussyBox shell.
>From BusyBox I can see that the mdadm array is being started correctly,
but that LVM is not.

To file this bug report, I attached the lucid-desktop-amd64.iso daily to
my VM and booted. From the live CD I installed mdadm and lvm2, verified
that LVM could be started correctly. So this seems to be a problem in
how the initramfs image is being created, not a problem with LVM.
