I've built you a kernel. It won't fix the errors, but it will give me
some clues as to what's going on. The kernel can be found here:

Once the kernel has been loaded and you have rebooted, if you could
issue the following command then unplug and re-plug your keyboard, it
should provide me with even more information.

echo 1 | sudo tee -a /sys/module/hid/parameters/debug

Once complete can you upload /var/log/kern.log please?
