Chuck has pinpointed the culprit; anytime the corresponding modem kernel module
of a base sound driver is loaded, it usurps the input and output of the original
base driver.

Please:

echo "snd-intel8x0m" | sudo tee -a /etc/hotplug/blacklist

and reboot.

The acpi=off boot parameter doesn't seem necessary here with respect to sound.

[As an aside, from inspecting the output of your lspci, I cannot deduce why
snd-atiixp is loaded. You don't appear to have hardware needing that driver.]
