/etc/hotplug/firmware.agent is an essential part of hotplug; the fact that it is
missing is why firmware loading isn't working for you.

This file is included when hotplug is initially installed; it must have been
deleted after installation.  Deleted configuration files are not automatically
restored when you reinstall a package.

sudo apt-get -o dpkg::options="--force-confmiss" --reinstall install hotplug
