[REVERT] [LUCID, MAVERICK] Reverting problematic upstream stable xhci patches in Lucid and Maverick

To recap a bit of history:
There were three USB3 xhci patches in the latest stable kernels for
Lucid, Maverick, and Natty.
We reverted the three patches in Natty, and it should be fine.
Upstream had a fix, which we applied to Lucid and Maverick, taking
it as a prestable patch.
That's how we got to this morning
We've learned today that there was an additional error in one of the
original three patches, requiring further fixing. Rather than take yet
another patch in hopes of fixing this, Let's revert all of the new
USB xhci patches in Lucid and Maverick that came from stable
- the three in the last stable update plus the one that was supposed
to fix them.
This leaves all our kernels in the same state regarding these
patches, and lets upstream sort it out before we take any of them.
The patches to be reverted are:
xhci: Fix full speed bInterval encoding.
USB: xhci - also free streams when resetting devices
USB: xhci - fix math in xhci_get_endpoint_interval()
USB: xhci - fix unsafe macro definitions
