[Precise][CVE-2013-2140 v2 0/1] xen/blkback: Check device permissions before allowing OP_DISCARD

I'm sending v2 of the backport of commit 604c499, which fixes
CVE-2013-2140.

This version implements the review comments from Seth and Stefan,
which prevent calling xen_vbd_translate() twice in the same code path
when a DISCARD operation is being executed.
