[CVE-2016-9794] ALSA: use-after-free in kill_fasync

Following this email I'm sending CVE-2016-9794 fix for precise, trusty,
vivid and xenial (yakkety has this fix already applied).  Except for
xenial (clean cherry-pick), the backports were taken from the upstream
3.12 stable backport done my Takashi Iwai (the original author).
