ACK: [PATCH Wily SRU] proc: revert /proc/<pid>/maps [stack:TID] annotation

Clean cherry-pick for Wily.  Already picked up in Xenial.

Note that this would be a good fix for T, U, and V also, but would
need backport work.

I recommend submitting this for consideration for 4.2 and 4.4 -stable.
