I've encountered it a few times and mostly chalked it up to strange quirks
introduced by proprietary programs. But then I encountered it in this:
http://www.linux-magazine.com/issue/62/KTools_Kat.pdf and I thought something
was seriously wrong. Like all other times, the pdf rendered correctly in xpdf
and kghostview.

I found this: https://bugs.kde.org/show_bug.cgi?id=110117 and it seems to be a
distrubution problem.
