With no software/system changes, last.fm streams are now working for me.
(I may have installed some other software, but I'm 99% sure none of it
was even vaguely related to media or KDE).  This suggests the last.fm
servers have now fixed some incompatibility.  The only thing that isn't
working is custom streams, but I think these are fixed in Amarok 1.4.6 -
