It was not related to the icon set, but I had desktop files for amarok and k3b (and others) in ~/.local/share/applications.
The one for amarok is named kde-amarok.desktop:
-rw------- 1 user user 389 2008-01-29 05:03 kde-amarok.desktop
-rw-r--r-- 1 user user 132 2007-09-29 19:53 k3b-usercreated.desktop


Strange, but I guess I've somehow managed to put it there myself (although it appears to have been updated in the meantime).
I'm closing the bugs as invalid. Thank you for making me look at it again.. :)
