This bug still exists on Kubuntu 8.04.
KMail 1.9.9
KDE 3.5.9
Linux 2.6.24-16-generic #1 SMP Thu Apr 10 12:47:45 UTC 2008 x86_64 GNU/Linux

It appears when one asks for bigger icons in the kontact/kmail toolbars. It seems to come from incomplete icon themes, for instance the icon named "mail_todo.png".
 
For CrystalSVG theme, a kmail crystalsvg iconset is in the folder /usr/share/apps/kmail/icons/. I am not sure whether it is better to modify  /usr/share/icons/crystalsvg/ or to modify /usr/share/apps/kmail/icons/crystalsvg/, but the following works:

Copy as root the icon kontact_todo.png into mail_todo.png in their respective size (32x32, 48x48 etc)
Example:
  /usr/share/icons/crystalsvg/32x32/actions/kontact_todo.png
into
  /usr/share/apps/kmail/icons/crystalsvg/32x32/actions/mail_todo.png
