Same error here (Ubuntu 9.10 + kdepim).

Removing ~/.local/share/akonadi did not solve the problem/

The most interesting parts of the report seem to be test 4 (InnoDB fails
to initialise the user database) and test 14:

[ERROR] Error message file '/usr/share/mysql/english/errmsg.sys' had only 480 error
  but it should contain at least 637 error messages.
  Check that the above file is the right version for this program!

Maybe the set of files distributed with mysql-server-core-5.0
(5.1.30really5.0.83-0ubuntu3) have been compiled using a different
version of mysql respect to what Akonadi (1.2.1-0ubuntu3) (or libqt4
-sql-mysql) have been compiled with?
