Further information

I have a workaround.

In ~/.kde/share/config/plasma-desktop-appletrc/ > [Containments] [16]
rests customIconSize.

It was as suspected set to 48. I changed it to 64. Logged out and back
in and the icons enlarged.

It appears that the icon size changer in SystemSettings does not, cannot
modify the config file.
