dolphin/system applications missing window controls?

Hello,

After a recent update (I'm running Intrepid), Dolphin and other KDE
windows (like System Settings) are missing their window controls and
cover the entire screen (covering and disabling the plasma dock). I can
only close them by using the menus, or using ctrl+q if there is not a
menu available. Any ideas how to fix this, or any where I can get more
information to diagnose? Other applications seem unaffected
(konversation, thunderbird, amarok, and firefox are all fine). I've been
running linux for about 10 years, so I'm no newbie, but I also am not
incredibly familiar with the inner workings of kde4 / plasma.
