A couple of weeks ago I started experiencing a problem where my 
integrated sound card was no longer enabled on boot.  KMix shows only 
"Dummy Output".  I have found a way to get it working after the system 
is up, doing the following:

$ killall -9 pulseaudio
$ rm -Rf ~/.config/pulse/
$ sudo alsamixer // Here I setup the sound card again and adjust volumes
$ sudo alsa force-reload
<logout and in again>

Sometimes this is all I need to do to bring the audio up, sometimes I 
have to repeat this process once or even twice before I can get the 
card to work.

What can I do to provide enough information to debug this issue?
