I have similar problems with a Dell Latitude D610 w. Intel
915GM. After booting without the external screen connected, trying to
switch to the external screen gives the result that both screens are
black, and then it's impossible to restore to the laptop screen
without restarting X.
The external screen seems to get some kind of signal that it can't
synchronize to.
Switching to the external screen w. Fn-F8 early in the bootup process
works, and gives a working X environment.
This is written in Xorg.log when I press Fn-F8 the first time:
(WW) I810(0): Detected possible lid operation, fixing up.
(II) I810(0): Display plane A is disabled and connected to Pipe A.
(II) I810(0): Display plane B is disabled and connected to Pipe B.
(II) I810(0): Enabling plane B.
(II) I810(0): Display plane A is now disabled and connected to Pipe A.
(II) I810(0): Display plane B is now enabled and connected to Pipe B.

(II) I810(0): PIPEBCONF is 0x00000000
(II) I810(0): Mode bandwidth is 78 Mpixel/s
(II) I810(0): maxBandwidth is 1152 Mbyte/s, pipe bandwidths are 420 Mbyte/s, 0 Mbyte/s
