Accessibility problem on Blueprint form

Like Matthew Revell, I'm on the Launchpad team, and I want to 
understand the exact problem you had.

1.  What browser were you using, and what screenreader software and
     version?

2.  Do you have JavaScript turned on?

3.  If you do, what happened when you activated the "Choose..." link on
     the registration form?
