I'm afraid Rosetta (or Launchpad, afaict) does not support adding this
kind of custom forms. We do have some infrastructure to get Ubuntu
contributors sign the Code of Conduct cryptographically, which is a
step, but will involve some PGP knowledge, etc. I'm filing a bug against
launchpad to discuss this idea. Some people might find it might have other
uses, not only for XaraLX.
Would having this form in the XaraLX website, and having it signed be a
prerequisite before one translator gets added to the XaraLX group work?
