I had a little bootfest to see what other vendors do/have done. I went 
back to RHL 4.2 (3.0.3 wouldn't boot, I have 4.0 but didn't bother).

Suse Server 9 doesn't ask about keyboard

Mandrake 10 Community does.
Red Hat does, from RHL 4.2 through FC2.
RHL 7.3 asks Model, Layout and Dead Keys.
UserLinux does
Progeny does not

One, I can't tell from my notes, offered the choice of three Dvorak 
keyboards.
