Brian,

Looks like the firmware loaded successfully.
Could you try 'hciconfig' to see if there is any hci device appears?
And try to restart bluetooth daemon
   sudo service bluetooth restart

Try to use the newer ath3k firmware if above steps won't work
   sudo cp /lib/firmware/ath3k-3.fw /lib/firmware/ath3k-3.fw.orig
   sudo cp /lib/firmware/ath3k-1.fw /lib/firmware/ath3k-3.fw
And then reboot

And be aware of that ath3k should not be loaded if you use this dkms, 
if your system will load ath3k after booting up, please add ath3k in the /etc/modprobe.d/blacklist.conf
