In the US, check out the RedPhone and TextSecure for Android from
http://whispersys.com - not open source yet (or even source code
available), but they say they will be soon.
On Linux/Windows/Mac, check out ZPhone, which integrates with many SIP
clients: http://zfoneproject.com/getstarted.html
I've been out of the loop for a while so there may well be other
products / projects out there, but that's what's on my radar :)
