The way to open a second instance from the Unity "icons" is by clicking it with your middle mouse button, or in most cases these days the mouse wheel.

Well I upgraded a pc that I don't really use to 11.04, and still don't like it. It fixes some of the issues that I had with it during beta(no tty, can't open 2nd instance of application with icon) but it seems you have to go through the search functions to find the application you want instead of Applications->Video->smplayer for example becomes : Applictions-> Type "smpl.."
