What we are doing right now in Kubuntu is that kde-guidance (=configuration 
tools) checks the PCI bus fro graphics cards at boot time. If a card is 
replaced or removed then an "apt reconfigure xserver" is run which 
regenerates /etc/X11/xorg.conf.

The way it works now is rather crude, but it works and is better than a broken 
X server.

Concerning recovering from a broken X server if the user configures a screen 
resolutino that their monitor can't display: This kind of recovery 
functionality should be part of the "recovery" boot option, and instead of a 
root shell it should offer a text menu to fix some basic things (like 
reverting to a previous version of /etc/X11/xorg.conf in this case).

A root shell would also be a menu option. :)
