So probably it should be
(1) choose which variety you want
(2) choose download/ShipIt/Amazon/etc (methods depending on the variety)
(3) fill out any form necessary (e.g. ShipIt request, Amazon order).

With JavaScript, (1) and (2) could be on the same page.
