Server Guide - Console Security Patch

Team,

Please review the following patch I have prepared for the Ubuntu Server
Guide, Security section.

Changes are noted in the commit comments, but to outline here, I've made
the following changes and additions.

- Added a section on Console Security
- The new section includes instructions on how to disable ctrl+alt+delete
key combinations, and usage of GNU GRUB passwords.
- Corrected minor typos in User Management section

As always, comments are encouraged.
