Yes, I agree that this must be the cause. As to whether it can be
avoided, I have to defer to Dustin...

Generally, just to the list. However, if the message you are replying
to copies in other people, or you are unsure whether the sender you
are reply to will be monitoring the list email, go ahead and use reply
to all.
The same basic guideline applies to all Ubuntu lists.
