I must have missed some of the replies as I don't seem to have a lot.
Perhaps they were sent offlist? My comments may be duplicated in that
case, but here are my thoughts:

First off, although relatively straightforward, Phil's first draft would
supply a whole schwack of good information as-is. I wouldn't change a
whole lot, but if I was going to change some things, it would be these:

1. I would add numbers to each question to facilitate our discussion of
the questions. It's easier to day "Question 1 might read better this
way..." rather than "The question about foo might read better this way.."

2. I would pursue which languages non-English Ubuntu users use if they
answer yes to that question.

3. Same with the language question about system documentation.

4. I think there might be a disconnect in the order of the questions
which have answers like "[Excellent	Pretty Good	Good enough Not
very good Very bad	Unusable	Never used it]" and questions which have
answers like "[1 (completely disagree) – 5 (completely agree)]"

In the first case, the 'best' answer will appear on the left and the
'worst' answer will appear on the right. In the second case, the order
is reversed in the sense that a 'completely disagree' is probably the
'worst' answer and a 'completely agree' is probably the 'best' answer.
Does that make sense?

That's about all I have at the moment, but I think we're off to a really
good start and can't wait to see it online!

Thanks

Jon
