First and foremost, we should strive for consistency across the
entire document.

Secondly, in a linux environment, it makes no sense to me at all
that there is a copyright notice on some pages. (example: #4)

That being said, I like #3 best.

However, and just to complicate things, I really like how
Gunnar Hjalmarsson is doing his "edited by" note, when
required:

     <name>Ubuntu Documentation Team</name>
   </credit>
